Jack Earl Perkins, Jr., age 39, entered into eternal rest on the evening of January 3, 2015 at UK Medical Center in Lexington, KY. He was born on December 8, 1975 at Wayne County Hospital in Monticello, KY. He lived in Monticello most of his life, but had recently moved to Liberty, KY to live on the farm as he had wanted to do for quite some time. Jack was preceded in death by his father, Jack Earl Perkins, Sr., step-father, Phillip Frederick, maternal grandparents, Modell and Cordell Poore, and paternal grandparents, Willie and Lillie Perkins.
He is survived by his special friend, Cindy South, his mother to whom he was her pride and joy, Imogene Frederick (Joe Gilpin) of Liberty, KY, three loving sisters, Jamie (Eddie) Bell, Jacqueline (Joel) Cunigan of Monticello, KY, and, Jessica (Dewayne) Riddle of Albany, KY. He is also survived by four nieces and three nephews, nieces, Juliet Perkins, Ashely (Chris) Loveless, Cody (Staci) Phillips, Brittany (Bradley) Albertson, Tyler Reynolds, Travis Bell, and Kristen Cunigan, one great niece and three great nephews, Elijah Loveless, Bryson and Bryleigh Albertson, and Carson Phillips; along with a host of family and friends who mourn his passing.
Jack was a kind, gentle soul and a friend to everyone he met. His laughter and quick wit will be missed by all who knew him.
The family will receive friends on Tuesday, January 6, 2015 after 5pm in the chapel of the funeral home. Funeral services will be held Wednesday, January 7, 2015 at 10am at the Hicks-Vaughn Funeral Home Chapel. Burial will be in the Ingram Cemetery.
